Before You Arrive
Preparing for Your First Appointment
- If you have received a welcome packet from our office, please
be sure to complete all the enclosed forms and bring them
with you to your first appointment.
- Please make every effort to provide our office with all
necessary insurance information prior to your first visit.
This will help us to make your appointment go quickly and
smoothly, as well as allow us to file your insurance claims
in a timely fashion.
- Please bring all insurance cards with you, as well as your
drivers license or other valid government issued photo
identification. For minor patients, the identification should
be of the parent accompanying the child.
- If your general dentist or dental specialist has referred
you for treatment, we will need a written referral. This referral
is not for insurance purposes, but is an important communication
tool between doctors. This way we know that any treatment
rendered will not interfere with the treatment plan that you
and your dentist have agreed upon.
- If your general dentist or dental specialist has referred
you for treatment, their office will often have x-rays that
we can use. (For wisdom teeth, the x-ray needs to be a panoramic
that is less than a year old.) Please try contacting your
general dentist prior to scheduling an appointment, to see
if they have appropriate x-rays. If they do, you could arrange
to pick them up or have them sent by mail to our office.
